CienOne Contract Privacy Policy

The Health Insurance Portability Accountability Act of 1996 (HIPAA) regulates the disclosure of consumers’ medical records. HIPAA requires that all individuals and organizations involved in a patient’s healthcare follow the HIPAA privacy practices. The Privacy Policy for CienOne outlines how your medical information will be protected and what your rights are with respect to the use and disclosure of your medical information. We will follow our Privacy Policy to help ensure that your personal medical information is protected.

DISCLOSURE We may disclose information to other healthcare organizations as required for the patient’s healthcare, medical billing and healthcare operations for quality improvement practices. We may provide information to your healthcare insurance carrier for claims processing. These healthcare providers and insurers are also obligated by HIPAA to keep your records private and confidential. We may be required by law to disclose medical information in response to a request from the government, law enforcement and or legal judicial orders. You hereby authorize us to disclose information to any friend or family member who is assisting with your medical care or providing financial arrangements on your behalf. Except as provided above, we will not disclose or release your information to anyone else without your written permission or other prior authorization.

REQUESTS You have the right to request a copy of your records and/or request changes to your records if you feel our records are inaccurate. NOTE: You may request a copy of or an amendment to your medical records with CienOne, LLC by submitting a written request. Please call our Medical Record Privacy Officer, at 941-639-9119 to obtain our address. Please provide contact information in your detailed written request. In addition you may request that we modify our Privacy Policy and/or that your medical information be released, or not be released, to another party. We will review the request and notify you of the actions to be taken. Any such request may be revoked by you at anytime with prior written notice to CienOne, LLC.

CHANGES TO OUR POLICY CienOne retains the right to make changes to its Privacy Policy. Clients will receive a copy of our Privacy Policy at the time services are rendered. Clients will not receive notice of subsequent changes to our Privacy Policy, but may request a written copy at anytime by contacting CienOne directly.

COMPLAINTS If you feel that CienOne has violated the privacy of your medical records, you have the right to file a written complaint with the United States Department of Health and Human Services. For guidance on how to file a complaint, you can visit www.HHS.gov.
